Job Purpose / Objective:
The Department of Internal Medicine Oncology is responsible for leading, managing, and enhancing the provision of specialized diagnostic, therapeutic, supportive, educational, and research services in internal oncology for patients with cancer. The department is tasked with delivering evidence-based, safe, effective, and integrated patient care in accordance with national and international accreditation standards. Additionally, it plays a key role in the training of internal oncology specialists through the postgraduate medical education (PGME) program.

Duties and Responsibilities:
Core Duties:
Clinical Services:
- Diagnose, treat, and follow up patients with solid tumors and hematologic malignancies according to national and international protocols and guidelines.
- Manage and implement systemic therapies, including chemotherapy, immunotherapy, targeted therapy, and hormonal treatments, ensuring strict adherence to patient safety principles.
- Manage complications arising from anti-cancer therapies and respond to oncologic emergencies.
- Provide supportive care and symptom management, including pain control, nutrition, infections, and palliative care.
- Provide specialized oncology consultations to OPD, IPD, emergency, ICU, surgical, and other related departments.
- Lead and supervise inpatient, outpatient, day-care chemotherapy, and supportive care services.
- Ensure evidence-based clinical decision-making and multidisciplinary treatment planning through Tumor Board discussions.
- Develop, implement, and update clinical protocols and guidelines for diagnosis and treatment of oncologic and hematologic conditions.
- Oversee accurate, complete, and timely documentation of clinical information, treatment plans, and patient outcomes.
- Lead and manage complex, rare, and challenging oncology cases and provide specialized interdepartmental consultations.
- Promote patient-centered care and ensure delivery of high-quality services with compassion and respect for patient rights.
- Maintain continuity of clinical services and effectively manage patient load within the department.
- Supervise and collaborate with fellows, trainees, nurses, and other healthcare professionals to ensure high-quality clinical care.
- Ensure smooth patient flow, timely interventions, and compliance with hospital quality standards.
- Evaluate clinical performance of the department based on patient outcomes and implement corrective actions to improve services.
- Provide technical guidance on safe handling and utilization of oncology medications and equipment.
- Perform on-call duties (primary and secondary as needed) according to the department and hospital schedule.
- Ensure timely completion of consultations and professional advice.
- Collaborate effectively with surgical, radiology, and palliative care departments to provide comprehensive, multidisciplinary care.
Educational Responsibilities (Postgraduate Training):
- Design, lead, implement, and supervise the Internal Medicine Oncology postgraduate training program according to approved policies and curriculum.
- Provide theoretical and practical training to trainees, fellows, and medical students, ensuring quality clinical and academic education.
- Implement competency-based curriculum and continuously assess skills and knowledge development.
- Conduct and supervise teaching ward rounds, case presentations, case-based discussions, journal clubs, MDT meetings, Tumor Boards, and Morbidity & Mortality reviews.
- Regularly evaluate trainees’ academic and skill progress, and monitor logbooks and other educational records.
- Collaborate with the PGME leadership in developing, revising, and updating the educational curriculum.
- Lead continuing medical education (CME/CPD) programs and ensure the team is updated on the latest scientific advances.
- Develop and implement mentoring and clinical guidance programs to foster a culture of learning and professional growth.
- Plan, execute, and supervise workshops, lectures, conferences, and scientific seminars to enhance the capacity of department staff and other hospital personnel.
- Integrate new methods, technologies, and treatments into educational programs and clinical services.
- Actively participate in ward visits, clinical case discussions, and scientific seminars.
- Ensure that the department’s educational programs align with the academic and training objectives of the hospital and Ministry of Public Health.
Implementation of Accreditation Standards:
- Ensure full compliance with accreditation standards, including patient safety and quality of care, chemotherapy safety and medication management, infection prevention and control (IPC), safe handling of cytotoxic drugs, and accurate clinical documentation.
- Lead clinical inspections, peer reviews, and complication audits.
- Manage risks, report adverse events, and implement corrective actions.
Administrative Responsibilities:
- Responsible for managing all professional and administrative affairs of the department, including supervision of recruitment, hiring, performance evaluation, and capacity building of all medical staff, including faculty, trainees, and fellows.
- Provide leadership, mentorship, and professional development opportunities for all department staff.
- Manage the department’s budget and resources to ensure appropriate allocation and cost efficiency.
- Collaborate with hospital leadership to ensure departmental compliance with regulations, accreditation standards, and quality indicators.
- Develop and organize 24-hour duty rosters for trainees, fellows, and nurses, including holidays and leave, based on scheduled shifts.
- Manage and actively participate in morning reports to ensure services provided in the previous 24 hours are delivered effectively.
- Oversee shift management, considering nurse and physician rotations, ensuring adequate staffing coverage across all internal oncology services.
- Develop and implement monthly, quarterly, annual, and strategic work plans for the department in alignment with hospital operational plans and Ministry of Public Health strategic objectives.
- Plan and organize daily departmental activities, ensuring compliance with clinical standards and diagnostic and treatment protocols.
- Manage staff affairs, including task assignments, duty rotations, and performance evaluations.
- Supervise the effective use of equipment, medications, and consumables.
- Ensure implementation of guidelines, standards, clinical protocols, and procedures to improve service quality.
- Monitor service quality, patient safety, and patient satisfaction.
- Perform on-call duties (primary and secondary) as needed according to the departmental schedule.
- Identify organizational needs and propose necessary positions in accordance with Ministry of Public Health norms.
- Participate in staff recruitment, performance evaluation, and professional development.
- Ensure continuing professional development (CPD) for doctors, nurses, and pharmacy staff.
- Maintain discipline, professional ethics, and accountability within the department.
- Participate in hospital-wide clinical supervision (Round Visits) and committee meetings, including DTC, Mortality Review, Quality Improvement, HMIS, and other sessions as convened by hospital administration.
- Submit monthly, quarterly, annual, and ad-hoc reports on departmental activities and achievements to the Hospital Chief Medical Officer.
- Perform other duties assigned by hospital leadership in accordance with hospital and Ministry of Public Health regulations and objectives.
Coordination Responsibilities:
- Participate in hospital committees and provide input in strategic planning, operational goals, and policy development.
- Promote a collaborative, supportive, and team-oriented work environment among faculty, staff, and trainees.
- Work closely with the Surgical Oncology, Radiotherapy, Pathology, Radiology, Hematology, and Pharmacy departments to provide comprehensive care to oncology patients.
- Collaborate with hospital committees on quality, patient safety, and medical ethics.
- Actively participate in multidisciplinary teams (MDTs) to provide integrated care.
- Maintain effective communication with hospital management and relevant institutions.
- Actively coordinate and participate in hospital committees related to patient safety, ethics, infection prevention (IPC), and quality improvement.
- Foster open and effective communication at all levels within the department to promote transparency, trust, and mutual respect.
Research and Academic Excellence:
- Design, implement, support, and oversee clinical and experimental research aligned with departmental clinical needs, hospital research priorities, and Ministry of Public Health policies.
- Promote and support clinical research in oncology, including multidisciplinary studies, case studies, scientific audits, and systematic patient data collection.
- Secure funding and resources for research projects and ensure transparent and effective implementation according to approved plans.
- Ensure adherence to research ethics at all stages (ERC/IRB), maintain patient confidentiality, protect patient rights, and follow professional ethical standards.
- Encourage and support department members to publish scientific articles and present research findings in national, regional, and international journals and platforms.
- Develop and expand scientific and research collaborations with universities, hospitals, research institutions, and cancer research centers domestically and internationally.
- Conduct regular weekly scientific meetings, conferences, and seminars on research methodologies to enhance staff knowledge, research skills, and scientific capacity.
- Continuously monitor, analyze, and evaluate the quality of research activities, outcomes, and achievements, providing recommendations for process improvement.
- Implement quality assurance and continuous improvement programs, prepare for internal and external audits and inspections, and ensure compliance with national and international accreditation standards.
- Continuously improve patient outcomes and reduce preventable complications by applying evidence-based practices and research findings.
- Promote a culture of research and academic excellence within the department and hospital.
Quality Assurance and Patient Safety:
- Promote a culture of patient safety and continuous quality improvement within the department.
- Ensure that internal oncology services comply with all health regulations, patient safety standards, and accreditation requirements.
- Monitor adherence to clinical quality standards and patient safety protocols within the department.
- Lead continuous quality improvement (CQI) initiatives and monitor departmental performance against established indicators.
- Collaborate with hospital administration and quality improvement committees to address issues related to patient safety, clinical care, and hospital-acquired infections (HAI).
Patient and Family Relations:
- Promote patient-centered care and ensure patients and their families are well-informed and involved in care decisions.
- Address and resolve concerns or complaints from patients and their families regarding care provided by the Internal Medicine Oncology Department.
- Work to enhance the hospital’s reputation as a provider of high-quality and compassionate care in the community.

Employment Requirements / Qualifications:
- Must be a citizen of Afghanistan.
- Age must be between 35 and 60 years.
- Educational Qualifications:
- Doctor of Medicine (MD) or equivalent degree from a recognized and accredited national or international university.
- Completion of a recognized specialty training in Internal Medicine Oncology (MD/DM/FCPS or an equivalent degree approved by the Ministry of Higher Education).
- Possession of a fellowship certificate in Internal Medicine Oncology is preferred.
- Work Experience:
- Minimum of 5–7 years of clinical experience in oncology post-specialization.
- At least 3 years of leadership or managerial experience in a teaching or specialized hospital.
- Verified experience managing a multidisciplinary team of medical professionals.
- Experience in teaching, mentorship, or supervision of medical students, trainees, or fellows.
- Experience leading clinical research, including securing research funding and publishing in reputable scientific journals.
- Licensing:
- Must hold a valid medical practice license from the Afghanistan Medical Council.
- Skills and Competencies:
- Proficiency in one of the national official languages (Dari or Pashto).
- Computer literacy in software relevant to the role.
- In-depth knowledge of hematologic malignancies, including recent advances in diagnosis and systemic therapies.
- Ability to teach, supervise, lead, and mentor trainees within the specialty training program.
- Awareness of healthcare regulations, accreditation standards, patient safety protocols, quality improvement initiatives, and clinical audit processes.
- Strong leadership, team management, decision-making, organizational, and problem-solving skills.
- Experience in clinical research, scientific publications, and clinical trials.
